Heather Ann San Juan

Heather Ann San Juan, nee Pawlak, age 51, of Elmhurst, IL passed away on April 1, 2025.  Born on March 16, 1974, Heather grew up in Oak Lawn, IL where she attended Oak Lawn High School and then went on to graduate from Lewis University with a Registered Nursing degree. Heather continued her education at Loyola University and became a Women’s Health Nurse Practitioner. In 2024, Heather was working on getting her MBA from the University of Illinois.

Heather was passionate about caring for people, specifically serving women and their healthcare needs. She was known for her intelligence and helping those around her with any sort of medical advice. She had an impressive career in the healthcare industry, working as a registered nurse, clinical consultant and educator, and an APP clinical liaison.

Most importantly, Heather had a zest for life and was a very proud mother of her twin children, Aidan and Caeley. She was close with family and friends and surrounded herself with them often. Heather loved gardening and animals. Her rabbits, Wrigley, Luna and Marshall, and her dogs, Jenna and Ivy, brought her great joy. Heather was a resourceful, handy and creative person and an excellent cook. You could feel the warmth and love she created in her home just by walking in the door. In 2018, Heather met her life partner, Jim Moskal. They developed a beautiful relationship and eventually merged their lives together. They shared a passion for music and traveling and enjoyed both with good friends and family.

Heather is survived by her children, Aidan and Caeley San Juan, her partner, Jim Moskal, her parents Sharon (Seyller) and David Pawlak, her siblings, Diane (Pawlak) and Steve Angelucci and JoAnne (Pawlak) Clemans, Niece Jill Angelucci and Nephews Torin Angelucci, Brandon Clemans, and Trent Clemans.

Visitation Monday, April 7, 2025, 3 p.m. until time of remembrances and eulogies 8 p.m. at Gibbons Funeral Home, 134 South York Road, (½ mile North of Saint Charles Road), Elmhurst.     To the family of Heather:
    I am a former coworker of Heathers. I worked with her at Novartis and one day she said to me, Carmen, let’s go to grad school together. Novartis pays for it. I looked at her and laughed and then realized she was actually serious.
    From that day, I applied to the University of Illinois and we started grad school together. We had many sleepless nights, stress filled exams (and some wine) through our first few semesters. She took a summer off, I took a few semesters off, and I am finally on my last semester.
    Heather always believed in me, encouraged me and supported me. She is the SOLE reason I went to grad school. She was also an amazing friend and coworker.
    On May 16, I will graduate with my MBA from University of Illinois. My degree will be dedicated to Heather in her honor. I am soon to be a grad school all because of Heather. I owe it all to her.
    She will be missed beyond what I can even express.
    I am so sorry for your loss…..I wish there were words of comfort I could provide. You are all in my thoughts and prayers.
